Comparing of the results in patients who underwent deep anterior lamellar keratoplasty and converted to penetrating keratoplasty

Purpose: Aimed to compare clinical outcomes of cases who underwent deep anterior lamellar keratoplasty (DALK) and converted to penetrating keratoplasty (PK) from DALK surgery
Materials and Methods: The records of 54 patients who the DALK surgery were planned and operated for different diagnosis between March 2013 and June 2015 were retrospectively analyzed. Patients were divided into two groups. 23 cases who converted to PKP due to Descemet’s membrane (DM) perforation at any stage of surgery were named as group 1 (PKP group), 31 patients whose surgery could be completed as DALK were named as group 2 (DALK group). Preoperative and postoperative follow-up results were evaluated in each groups.
Results: Corrected distance visual acuity (CDVA) according to baseline in both groups increased in the postoperative period. But there was not statistically significant difference in the rates of CDVA increase between the groups (p: 0.142). The mean value of astigmatism that obtained with corneal topography was 5.8±2.3 dioptrics in group 1, and was 5.4±1.8 dioptrics in group 2 at last examination. The difference was no statistically significant between groups (p: 0.430). The groups were not statistically different regarding the postoperative pachymetry (p: 0.453). The grafts in all 54 (%100) patients were clear at the last examination, postoperatively. There were no statistically significant differences between the groups in terms of the postoperative complications.
Conclusion: The similar clinical outcomes were obtained in our study for the patients who underwent DALK and converted to PKP from DALK.
Derin anterior lameller keratoplasti yapılan vakalar ile penetran keratoplastiye dönüş yapılan vakaların sonuçlarının karşılaştırılması

Amaç: Derin anterior lameller keratoplasti (DALK) cerrahisi uygulanmış hastalar ile DALK cerrahisinden penetran keratoplasti (PKP) cerrahisine dönüş yapılan olguların klinik sonuçlarının karşılaştırılması amaçlandı.
Gereç ve Yöntem: Mart 2013 - Haziran 2015 tarihleri arasında farklı endikasyonlarla DALK cerrahisi planlanan ve opere edilen 54 hastanın kayıtları retrospektif olarak incelendi. Hastalar iki gruba ayrıldı. Cerrahinin herhangi bir aşamasında desme membranı (DM) perforasyonu nedeniyle PKP’ye dönüş yapılan 23 olgu grup 1 (PKP grubu), ameliyatı DALK olarak tamamlanabilen 31 olgu ise grup 2 (DALK grubu) olarak adlandırıldı. Heriki grupta ameliyat öncesi ve sonrasındaki takip sonuçları değerlendirildi.
Bulgular: Düzeltilmiş uzak görme keskinliği (DUGK) heriki grupta da ameliyat öncesine göre, postoperatif dönemde artmıştır. Fakat gruplar arasında DUGK artış oranlarında istatistiksel olarak anlamlı fark saptanmamıştır (p: 0.142). Son muayenedeki korneal topografi ile elde edilmiş ortalama astigmatizma değeri grup 1’ de 5.8±2.3 dioptri, grup 2’ de ise 5.4±1.8 dioptri idi. Gruplar arasında istatistiksel anlamlı fark yoktu (p: 0.430). Gruplar arasında postoperatif pakimetri değerlerinde istatiksel olarak anlamlı fark yoktu (p: 0.453). Heriki grupta da ameliyat sonrası dönemde yapılan son muayenede 54 (%100) olgunun tamamında greftler saydam olarak gözlendi. Postoperatif komplikasyon oranlarında istatiksel olarak anlamlı fark yoktu.
Sonuç: Çalışmamızda DALK yapılan olgularla, DALK’tan PKP’ye dönüş yapılan olgular arasında ameliyat sonrası dönemde benzer klinik sonuçlar elde edilmiştir.
