Use of contact lenses on eyes with severe keratoconus: Long-term results

lenses on severe keratoconic eyes.
Materials and methods: Severe keratoconic eyes with RGP contact lenses were evaluated retrospectively.
Results: Long-term follow up results in RGP contact lenses, applied to 59 eyes of totally 42 cases (25 women, 17 men) with the diagnosis of severe keratoconus, were analysed. Follow-up period was 3 to 12 years (mean 6,57±3,60). Mean age at he first exam was 25,88±9,10 years; mean corneal curve K1 was 6,49±0,40 mm. and K2, 5,91±0,40 mm. Mean visual acuity with speclacles and contact lenses were 0,26±0,10 lines(0,63±0,20 logMAR) and 0.66±0,20 lines (0,20±0.10 logMAR) respectively. The difference between visual acquities were statistically significant (p=0,0001). At the last visit, the mean visual acuity with RGP contact lenses was 0,68±0,20 lines (0,19±0,10 logMAR). There was no statistically significant difference in visual acuity between first and last examination with contact lenses (p=0,32). During long-term follow up period, apical scarring developed in 17 eyes. Only 2 eyes of one patient needed penetrating keratoplasty after 6 years of RGP contact lens use.
Discussion: On severe keratoconic eyes, RGP contact lenses should be tried before penetrating keratoplasty.
İleri keratokonuslu olgularda kontakt lens uygulaması: Uzun süreli takip sonuçlarımız

Yöntem: SGG kontakt lens uygulanan ileri keratokonuslu gözler retrospektif olarak incelendi.
Sonuçlar: İleri keratokonus tanısı konup gaz geçirgen kontakt lens uygulanan 25 kadın 17 erkek 42 hastanın 59 gözü çalışma kapsamına alındı. Hastalar 3 ile 12 yıl (ortalama 6,57±3,60) takip edildi. İlk muayenede yaş ortalaması 25,88±9,10; K1 değeri ortalaması 6,49±0,40mm, K2 değeri ortalaması 5,91±0,40mm. idi. Gözlük ve kontakt lens ile ortalama görme keskinlikleri 0,26±0,10 (0,63±0,20 logMAR) ve 0.66±0,20 (0,20±0.10 logMAR) bulundu. Görme keskinlikleri arasındaki fark istatistiksel olarak anlamlıydı(p=0,0001). Son muayenede SGG kontakt lens ile ortalama görme keskinliği 0,68±0,20 (0,19±0,10 logMAR) idi. İlk ve son muayenelerde kontakt lens ile alınan görme keskinlikleri arasında istatistiksel olarak anlamlı bir fark yoktu(p=0,32). Uzun takip süresi sonunda 17 gözde apikal skar gelişti. Sadece bir hastanın 2 gözüne 6 yıl SGG kontakt lens kullanımından sonra keratoplasti gerekti.
Tartışma: İleri keratokonusu olan gözlerde SGG kontakt lensler, keratoplasti öncesi mutlaka düşünülmelidir.
