Descemet Stripping Automated Endothelial Keratoplasty for Treatment of Pseudophakic Bullous Keratopathy

Material and Methods: Twenty eyes of 20 patients who underwent DSAEK with a diagnosis of PBK in our clinic between September 2010 - January 2012 were included in this study. Pre- and postoperative best-corrected visual acuity (BCVA), refraction values, endothelial cell density, graft position were analyzed. Complications and treatments were recorded.
Results: Eleven female and 9 male 20 patients’ mean age was 71.44±7.55 years (56-83 years), and mean follow-up time was 11.35±3.90 months (3-16 month). Preoperative mean BCVA was 0.04±0.044, 12. months BCVA was 0.55±0.22 (p=0.001). Mean endothelial cell density was 1508.42 ± 254.82 cell/mm² at 12. months. Dislocation of graft lenticule occured in five patient. Graft failure occured in one patient.
Conclusion: DSAEK is an alternative technique with a more rapid rehabilitation due to PK for the treatment of endothelial diseases although a steep learning curve and different complications.
Psödofakik Büllöz Keratopati Tedavisinde Descemet Soymalı Otomatize Endotelyal Keratoplasti

Materyal ve Metod: Kliniğimizde Eylül 2010 – Ocak 2012 tarihleri arasında PBK tanısı ile DSAEK uygulanan 20 hastanın 20 gözü çalışmaya alındı. Hastaların ameliyat öncesi ve sonrası en iyi görme keskinliği (EİGK), refraksiyon değerleri, endotel hücre yoğunluğu, greftin konumu değerlendirildi. Komplikasyonlar ve uygulanan tedaviler kaydedildi.
Bulgular: Onbiri kadın, 9’u erkek 20 hastanın ortalama yaşı 71.44±7.55 yıl (56-83 yıl), ortalama takip süresi 11.35±3.90 ay (3-16 ay) idi. Ameliyat öncesi EİGK ortalaması 0.04±0.044 iken 12. ayda 0.55±0.22 idi (p=0.001). Endotel hücre yoğunluğu 12. ayda ortalama 1508.42 ± 254.82 hücre/mm² idi. Beş hastada greft ayrışması izlendi. Bir hastada greft yetmezliği görüldü.
Sonuç: DSAEK, öğrenme eğrisi dik, farklı komplikasyonları olan bir teknik olmasına rağmen daha hızlı rehabilitasyon sağlanması nedeni ile endotel hastalıklarının tedavisinde PK’ya alternatif bir tekniktir.
