The evaluation of tear function tests and the lower tear meniscus height in keratoconus patients

Material-Metods: Thirty-eight eyes (group 1) of 21 keratoconus patients and 36 eyes (group 2) of 18 healthy subjects were included in this prospective study. Both groups were performed corneal topographic and keratometric measurements, tear break up time (T-BUT) and Schirmer tests, measurement of the lower tear meniscus height (LTMH) with the anterior segment optic coherence tomography (OCT) after the ophthalmologic examination. The values obtained from both groups were compared and evaluated for statistical significance and reliability.
Results: There were no statistically significant difference between age and gender in 2 groups (p>0.05). The mean Schirmer test values were 14.87±8.9 mm, and 16.77±8.1 mm in group 1 and group 2 respectively (p=0.367). There was not any correlation between the keratometric power and the Schirmer test in group 1 and group 2 (grup 1: r=0.114, p=0.548, grup 2: r=0.151, p=0.972). The mean TBUT value was 12.83±7.3 sec. in group 1, and 18.25±8.5 sec. in group 2 (p=0.018). There was a negative correlation between the keratometric power and the TBUT in grup 1, however there was no correlation in group 2 (grup 1: r=0.717, p=0.001, grup 2: r=0.235, p=0.212). The mean LTMH was 265.30±112 μm in group 1 and 313.29±167 μm in group 2 (p=0.151). There were no correlation between the keratometric power and the LTMH in group 1 and group 2 (grup 1: r=0.001, p=0.997, grup 2: r=0.318, p=0.130).
Conclusions: In this study it was shown that keratoconus patients have normal tear volume, but reduced tear film stability according to normals and this reduction is relate to the progression of keratoconus.
Keratokonus hastalarında gözyaşı fonksiyon testleri ve alt gözyaşı menisküs yüksekliğinin değerlendirilmesi

Gereç-Yöntem: Prospektif olarak yapılan bu çalışmaya 21 keratokonus hastasının 38 gözü (grup 1) ile 18 sağlıklı bireyin 36 gözü (grup 2) dahil edildi. Her 2 gruba da rutin oftalmolojik muayeneyi takiben korneanın topografik ve keratometrik ölçümleri, gözyaşı kırılma zamanı (GKZ) ve Schirmer testleri, ön segment optik koherans tomografisi (OCT) ile alt gözyaşı menisküs yüksekliği (AGMY) ölçümleri yapıldı. Her 2 gruptan elde edilen değerler istatistiksel olarak karşılaştırılarak, anlamlılık ve güvenilirlik bakımından incelendi.
Bulgular: Yaş ve cinsiyet açısından 2 grup arasında istatistiksel açıdan anlamlı bir farklılık yoktu (p>0.05). Ortalama Schirmer testi değerleri grup 1’de ve grup 2’de sırası ile 14.87±8.9 mm ve 16.77±8.1 mm idi (p=0.367). Grup 1 ve grup 2’de keratometrik güç ile Schirmer testi arasında korelasyon saptanmadı (grup 1: r=0.114, p=0.548, grup 2: r=0.151, p=0.972). Ortalama GKZ değeri grup 1’de 12.83±7.3 sn., grup 2’de 18.25±8.5 sn. idi (p=0.018). Keratometrik güç ile GKZ arasında grup 1’de negatif bir korelasyon gözlenirken, grup 2’de korelasyon gözlenmedi (grup 1: r=0.717, p=0.001, grup 2: r=0.235, p=0.212). Ortalama AGMY grup 1’de 265.30±112 μm, grup 2’de 313.29±167 μm idi (p=0.151). Grup 1 ve grup 2’de keratometrik güç ile AGMY arasında herhangi bir korelasyon saptanmadı (grup 1: r=0.001, p=0.997, grup 2: r=0.318, p=0.130).
Sonuç: Bu çalışmada keratokonus hastalarında gözyaşı hacminin normal olduğu, fakat gözyaşı film tabakası stabilitesinin normale göre azaldığı ve bu azalmanın keratokonusun ilerleme derecesi ile orantılı olduğu gözlendi.
