The Comparison Of The Effect of Trabeculectomy With Mitomycin C On Corneal Hystresis In Primary Open-angle Glaucoma And Pseudoexfoliative Glaucoma

Material and methods: 21 eyes of 21 cases with PXG (12 male, 9 female, mean age: 67,8±5,6) and 20 eyes of 20 cases with POAG (12 male, 8 female, mean age: 66,7±6,1) were included to our prospective study. All of the eyes undergone trabeculectomy with mit C. CH was measured preoperatively and at the first postoperative week and month by using ocular response analyser (ORA). The differences of CH pre and postoperatively between the groups were analyzed statistically by using Mann-Whitney U, Wilcoxon and chi-square tests.
Results: There were no statistically significant differences of CH (p=0,12), corneal resistance factor (CRF) (p=0,054), corneal compensated intraocular pressure (IOPcc) (p=0,16) and Goldmann-correlated intraocular pressure (IOPg) (p=0,07) between PXG and POAG cases preoperatively. In both PXG and POAG cases, the mean CH increased significantly at the first postoperative week and month (POAG: 1st week p=0,01 1st month p=0,001; PXG: 1st week p=0,01, 1st month p=0,001). But no statistically significant differences were found between the changes of preoperative and postoperative mean CH values between the two groups (1st week p=0.88, 1st month p=0.084).
Conclusion: In spite of the significant increases in CH at the first postoperative day and month in both POAG and PXG cases, there were no significant differences between the changes of preoperative and postoperative mean CH values between the two groups. The postoperative increase in CH in POAG and PXG cases was thought to be correlated with the decrease in intraocular pressure.
Primer Açık Açılı Glokom Ve Psödoeksfoliatif Glokomda, Mitomisin C’li Trabekülektominin Korneal Histerezis Üzerine Olan Etkisinin Kıyaslanması

Materyal ve metod: Mit C’li trabekülektomi cerrahisi planlanan PEG’lu 21 olgunun (12 erkek, 9 kadın ortalama yaş: 67,8±5,6) 21 gözü ve PAAG’lu 20 olgunun (12 erkek, 8 kadın ortalama yaş: 66,7±6,1) 20 gözü prospektif çalışmamıza dahil edildi. KH ölçümleri, cerrahi öncesinde ve cerrahi sonrası 1. hafta ve 1. ayda Oküler cevap analizörü ORA) kullanarak yapıldı. Cerrahi öncesi ve sonrası değerler, ayrıca gruplar arası farklılıklar Mann-Whitney U, Wilcoxon ve ki-kare testi ile istatistiksel olarak analiz edildi.
Bulgular: PAAG ve PEG grupları arasında cerrahi öncesi KH (p=0,12), korneal rezistans faktörü (KRF) (p=0,054), korneal kompanse göz içi basıncı (GİBcc) (p=0,16) ve Goldmann uyumlu göz içi basıncı (GİBg) (p=0,07) değerleri arasında anlamlı farklara rastlanmadı. Gerek PAAG gerekse PEG’lu olgularda, cerrahi sonrası 1.hafta ve 1.aydaki ortalama KH değerleri, cerrahi öncesi değerlere oranla istatistiksel olarak anlamlı ölçüde yüksek bulundu (PAAG: 1.hafta p=0,01 1.ay p=0,001; PEG: 1.hafta p=0,01, 1.ay p=0,001). Ancak her iki grup arasında, cerrahi sonrası 1.hafta ve 1.aydaki KH değerlerindeki artışlar arasında anlamlı fark gözlenmedi (1.hafta p=0,88, 1.ay p=0,084).
Tartışma: PAAG ve PEG’lu olgularda, cerrahi sonrası 1.gün ve 1.aydaki ortalama KH değerlerinde, cerrahi öncesi değerlere oranla anlamlı ölçüde artış görülse de, bu artışlarda her iki grup arasında anlamlı farka rastlanmadı. PAAG ve PEG olgularında postoperatif dönemde KH’ deki artışın, GİB’daki düşmeye bağlı olduğu düşünüldü.
