Surgical outcome of autogenic hard palate grafts in lower eyelid reconstruction

Purpose: To evaluate the surgical outcome of autogenic hard palate mucosal grafts (HPMG) in full thickness lower eyelid reconstruction following tumor excision.
Material and Method: The medical records of patients who had full thickness lower eyelid tumour excision between 2005 and 2009 were retrospectively reviewed. Seven patients who had HPMG for posterior lameller reconstruction were included in the study. Graft complications like necrosis, contraction, excessive mucous discharge and potential problems in the graft harvesting site and technique were evaluated.
Results: Seven eyelid reconstructions with HPMG were performed on 4 female, 3 male patients with an age range of 48 to 71 years at surgery. Patients were followed for 6 to 56 months. Two patients had inferior eyelid retraction of which one of them was severe and one was mild. None of the patients had HPMG necrosis. Donor site haemorrhage occured in two patients which was controlled with direct pressure and cauterization in the early postoperative period.
Conclusions: Hard palate mucosal grafts have been the golden standard for the reconstruction of posterior lameller eyelid defects. The main problem about these grafts is that the oral cavity is an unusual region for ophthalmologists with potential complications arising from the hard palate. Comprehension of the anatomy, proper surgical tecnique and post operative care can decrease the rate of these complications.
Alt kapak rekonstrüksiyonunda otojen sert damak mukoza grefti uygulama sonuçları

Amaç: Tümör eksizyonu sonrası tam kat alt kapak defektlerinin rekonstrüksiyonunda sert damak mukoza grefti (SDMG) kullanımının değerlendirilmesi.
Gereç ve Yöntem: 2005-2009 yılları arasında, alt kapakta tümör nedeniyle tam kat eksizyon uygulanan hastaların kayıtları retrospektif olarak değerlendirildi. Arka lamel rekonstrüksiyonunda SDMG kullanılan 7 hasta çalışmaya dahil edildi. SDMG’inde kontraksiyon, nekroz, greft kaynaklı sekresyon artışı, sert damaktan greft alınma metodu ve alınma yeri ile ilgili yaşanabilecek sorunlar değerlendirildi.
Bulgular: Ameliyat sırasındaki yaşları 48- 71 arasındaki 7 hastaya (4 kadın, 3 erkek) tümör eksizyonu sonrasında SDMG ile arka lamel rekonstrüksiyonu uygulandı. Ameliyat sonrası takipleri 6-56 ay arasında olan hastaların birinde ciddi, bir diğerinde ise hafif derecede alt kapak retraksiyonu gelişti. Sert damak mukoza greftinde nekroz problemi yaşanmadı. İki hastada ameliyat sonrası sert damak bölgesinde kanama oldu, baskılı pansuman ve hafif koterizasyon ile kanama kontrol altına alındı.
Sonuç: SDMG alt kapakta arka lamel rekonstrüksiyonlarında altın standart olarak kullanılmaktadır. Bu greftlerle ilgili ana problem ağız içinin göz doktorlarının alışık olmadıkları bir bölge olması, ve sert damak bölgesine ait komplikasyonların olabilmesidir. Anatomiye daha hakim olunması, uygun cerrahi teknik ve operasyon sonrası bakımla bu komplikasyonlar azaltılabilir.
