Postmenapozal Osteoporoz Hastalarında Nöropatik Ağrı; Uyku Kalitesi, Yaşam Kalitesi ve Depresyon Üzerine Etkileri
Gülseren Dost Sürücü, Semra Teker, Dilay Eken-GedikAdıyaman Üniversitesi Tıp Fakültesi, Fiziksel Tıp ve Rehabilitasyon Ana Bilim Dalı, AdıyamanAmaç: Bu çalışmada amacımız Postmenopozal Osteoporoz (OP) hastalarında nöropatik ağrının sıklığını, uyku kalitesi, yaşam kalitesi, depresyon üzerine olan etkilerini ve bu bulguların birbiri ile ilişkisini araştırmaktır.
Gereç ve yöntem: Çalışmamıza 40-75 yaş arası 250 postmenapozal OP’si olan kadın hasta alındı. Hastalar sosyodemografik özellikler ve risk faktörleri gibi tanımlayıcı özellikler içeren hasta bilgi formu, Avrupa Osteoporoz Derneği Yaşam Kalitesi Anketi (QUALEFFO-41), Beck Depresyon Ölçeği (BDÖ) ve Pittsburgh Uyku Kalite İndeksi (PUKİ) soru formlarını cevapladı. Nöropatik ağrının şiddeti S-LANSS (Self-administered Leeds Assessment of Neuropathic Symptoms and Signs) ağrı skalası ile değerlendirildi.
Bulgular: Hastaların ortalama yaşı 58,57±7,3 yıldı. S-LANNS ölçeğine göre hastaların %44,8’ inde nöropatik ağrı eşlik etmekteydi. Nöropatik ağrısı olan vakaların olmayanlara göre L1-L4 T (p=0,030) ve L1-L4 KMY (p=0,008) skor ortalamalarının istatistiksel açıdan anlamlı seviyede daha düşük olduğu bulundu. Nöropatik ağrısı olan vakaların olmayanlara göre QUALEFFO-41 Toplam puan ve alt ölçek skorlarının, BDÖ skor ortalamalarının, PUKİ ve alt ölçekleri skorlarının istatistiksel açıdan anlamlı seviyede daha yüksek olduğu bulundu.
Sonuç: OP ağrısı genel olarak nosiseptif karakterde bilinmesine rağmen, hastaların önemli bir kısmına nöropatik ağrı komponenti eşlik etmektedir. OP hastalarında nöropatik ağrı komponenti; bozulmuş uyku, yaşam kalitesi ve depresyonla ilişkilidir.
Neuropathic Pain in Postmenopausal Osteoporosis Patients; Effect on Sleep Quality, Quality of Life and Depression

Materials and Methods: 250 women with postmenopausal osteoporosis were included in the study. Patient information form containing descriptive features such as sociodemographic characteristics and risk factors, European Osteoporosis Society Quality of Life Questionnaire (QUALEFFO-41), Beck Depression Scale (BDS) and Pittsburgh Sleep Quality Index. (PUKI) answered the questionnaires. The severity of neuropathic pain was assessed using the S-LANSS (Self-administered Leeds Assessment of Neuropathic Symptoms and Signs) pain scale. Results: . According to the S-LANNS scale, 44.8% of the patients were accompanied by neuropathic pain. The mean scores of L1-L4 T (p=0.030) and L1-L4 BMD (p=0.008) were found to be statistically significantly lower in patients with neuropathic pain compared to patients without neuropathic pain. It was found that QUALEFFO-41 total score and subscale scores, BDI score averages, PUKI and subscale scores of the patients with neuropathic pain were statistically significantly higher than those without neuropathic pain.Conclusion: Although OP pain is generally known with a nociceptive character, a significant number of patients are accompanied by a neuropathic pain component. Neuropathic pain component in OP patients; Impaired sleep and quality of life are associated with depression.
