Postmenopozal osteoporoz tedavisinde uzun dönem bifosfonat kullanımı ve görülebilen yan etkiler

İstanbul, TürkiyeBifosfonatlar ostoporozun önlenmesi ve tedavisinde en sık kullanılan, genelde iyi tolere edilebilen, güvenilir ilaçlardır. Ancak belirli yan etkileri de tanımlanmıştır. Gastrointestinal sistem ve böbrek fonksiyonları üzerinde oluşturduğu olumsuz etkileri ve neden olduğu akut faz reaksiyonu uzun yıllardır iyi bilinmektedir. Bir yan etki olarak atrial fibrilasyon ise ilk olarak zoledronik asit çalışmasında tanımlanmış ancak takip eden çalışma ve analizlerde bifosfanat ile ilişkisi kesinleştirilememiştir. Aynı şekilde bifosfonat kullanımı ile çene osteonekrozu, özofagus kanseri ve atipik femur kırığı arasında olası düşük riskli bir ilişki üzerinde durulmaktadır. Ancak konular ile ilgili prospektif randomize kontrollü çalışmalar mevcut değildir.

Cerrahpaşa Medical Faculty,department Of Physical Medicine And RehabilitationBisphosphonates are the most commonly used drugs for the prevention and treatment of osteoporosis. Bisphosphonates are generally well tolerated and safe. Nevertheless, adverse events have been noted. Effects on the gastrointestinal tract and kidney function are well recognized, as are transient acute-phase reactions. Atrial fibrillation was first identified as a potential adverse event in a zoledronate trial, but subsequent trials and analyses failed to substantiate an association with bisphosphonates. Likewise a possible association between bisphosphonate use and osteonecrosis of the jaw, esophageal cancer and subtrochanteric fractures has also been suggested. However, the risk appears to be low, with no evidence from prospective RCTs of a causal association.
