Pregnancy Assocated Osteoporosis Leading to Vertebral Compression Fracture

Objective: To describe a patient with low back pain developed in the postpartum period and diagnosed as having vertebral compression fracture due to pregnancy and lactation associated osteoporosis.
Method: A 28-year old woman presented with complaints of low back pain that started two months after her first delivery. Laboratory tests including bone mineral density (BMD), biochemical evaluation and lomber spinal MRI were ordered.
Results: Lomber spine BMD was extremely decreased: L2-4: 0,685 g/cm2, T-score -3.9, Z-score -3.9. MRI revealed a compression fracture of T12 vertebra. The patient was investigated in terms of all possible risk factors and hormonal pathology causing osteoporosis and no abnormality was found.So She was diagnosed as having pregnancy and lactation associated osteoporosis. Cease of the lactation and the treatment with teriparatide was recommended to the patient but she refused both of them.
Discussion: Clinicians should take into account of pregnancy and lactation associated osteoporosis, when evaluating patients with low back pain in early postpartum period.

Amaç: Postpartum dönemde bel ağrısı şikayetleri olan ve gebelik ve emzirme ilişkili osteoporoz sebebiyle vertebra kompresyon fraktürü tanısı konulan bir hasta sunulmuştur.
Metod: 28 yaşında kadın hasta doğumdan 2 ay sonra başlayan bel ağrısı şikayetleri ile başvurdu. Hasta kemik mineral yoğunluğu (KMD), biyokimyasal incelemeler ve lomber vertebra Manyetik Rezonans Görüntüleme (MRG) ile değerlendirildi.
Sonuç: Lomber vertebra KMD değerinde önemli ölçüde düşme saptandı. L2-4: 0,685 g/cm2, T-score -3.9, Z-score -3.9. MRG’de T12 vertebrada kompresyon fraktürüne rastlandı. Hasta osteoporoz açısından tüm olası risk faktörleri ve sebep olabilecek hormonal bozukluklar yönünden değerlendirildi ve herhangi bir anormallik bulunmaması üzerine gebelik ilişkili osteoporoz tanısı kondu. Hastaya emzirmeyi sonlandırması ve teriparetid tedavisi önerildi ancak hasta kabul etmedi.
Tartışma: Erken postpartum dönemde karşımıza çıkan bel ağrısında gebelik ve emzirme ilişkili osteoporoz konusunda klinisyenler dikkat etmelidirler.
