Intramedullary spinal cord metastasis is very rare. We present the case of a 32-year-old male patient with a 2-month history of bilateral lower extremity weakness. Initial Magnetic Resonance Imaging of the dorsal spine showed abnormal thickening of the dorsal spinal cord at the level of the Thoracal 1-2 vertebral bodies. F-18 FDG-PET/CT images revealed a hypermetabolic lesion in the spinal cord at Thoracal 1-2 vertebral level. This patient had an intramedullary spinal cord metastasis, a rare form of metastatic disease, secondary to malignant mesenchymal tumor.
