Herein, such a patient is described 50-years-old man who was hospitalized to gastroenterology clinic due to fatigue, nausea and fever for the last 10 days. Computed tomography of abdomen revealed abscess in the right retroperitoneal space involving perinephritic region. Laparotomy and retroperitoneal exploration were performed immediately and pus was drained. A ruptured retrocecal appendix was confirmed as the cause of abscess. Perforated acute appendicitis can occasionally manifest as complicated retroperitoneal abscess without remarkable abdominal symptoms; thus, it is necessary to maintain a high index of suspicion in a patient with symptoms of retroperitoneal infection.
