Bluetooth etkin cilt sensörü ve akıllı telefon uygulaması ile vücut sıcaklığı ölçme yönteminin aksiller dijital termometre ile karşılaştırılması

GİRİŞ ve AMAÇ: Çocuklarda ateş ölçümünün en doğru ve en güvenilir yöntemi yıllardır araştırılan bir konudur. Son yıllardaki teknolojik gelişmelerle akıllı telefonlar kullanılarak, non-invaziv sürekli ateş ölçümü yapılabilmesi, ölçülen ateş değerlerinin kaydedilmesi ve ateş yüksekliği durumunda uyarı verilebilmesi mümkün hale gelmiştir. Ancak bu yöntemle yapılacak ateş ölçümlerinin doğruluğu ile ilgili yeterli bilimsel veri şuan için mevcut değildir. Bu çalışmanın amacı akıllı telefon uygulaması ile yapılan vücut sıcaklık ölçümlerini aksiller dijital termometre ile karşılaştırmaktır.
YÖNTEM ve GEREÇLER: Çocuk acil servisimize 1 Temmuz-30 Eylül 2017 tarihleri arasında ateş yüksekliği ile başvurup gözleme alınan tüm hastalar çalışmaya dahil edildi. Hastaların demografik özellikleri, acil servise geliş tanıları ve acil serviste izlemleri boyunca saatlik olarak hem dijital aksiller termometre ile hem de akıllı telefon uygulaması ile aynı anda ölçülen ateş değerleri kaydedildi. Her iki yöntemle ölçülen bu değerler birbirleriyle karşılaştırıldı.
BULGULAR: Çalışma süresi boyunca 53 hastadan 234 ölçüm yapıldı. Hastaların 30’u (%57) erkekti, yaş ortalamaları 58±51 aydı ve 11’i (%21) 1 yaş altı hastalardı. Sırasıyla üst solunum yolu enfeksiyonu (20, %38) ve akut gastroenterit (12, %23) hastalara en sık konulan tanılardı. Aksiller ölçümlerde ateş yüksekliği saptanan hastaların çoğunda akıllı telefon uygulaması ile yapılan ölçümlerde de ateş yüksekliği tespit edilirken, iki yöntem arasında ateşi saptamada anlamlı bir fark yoktu (p=0.754). Tüm aksiller ölçümlerin ortalaması 37,07±0,85 oC iken, akıllı telefon uygulaması ile yapılan ölçümlerin ortalaması 37,16±0,89 oC olarak hesaplanıp iki değerin güçlü pozitif korele olduğu saptandı (r=0.874, p<0.001). Ateşli dönem aksiller ölçüm ortalaması 38,14±0,56 oC, akıllı telefon uygulaması ölçüm ortalaması 38,24±0,55 oC idi ve benzer şekilde ateşli dönem de güçlü pozitif korelasyon söz konusuydu (r=0.745, p<0.001). Ayrıca infantil dönemde de benzer sonuçlara ulaşıldı.
TARTIŞMA ve SONUÇ: Akıllı telefon uygulaması ile ateş ölçme yöntemi güvenilir, her yerde ve tüm yaş gruplarında kullanılabilecek üstün bir yöntem gibi görünmektedir ancak bu yöntemin rutin kullanıma girmesi için daha çok çalışmaya ihtiyaç vardır.

Comparison of axillary digital thermometer with smart fever measurement

INTRODUCTION: The best method of fever measurement in children has been investigated for many years. It has become possible to make non-invasive continuous fever measurement by using smart phones, to record the measured fever values and to give warning in case of fever with the technological developments in recent years. Nevertheless, there is not data on the accuracy of fever measurements to be made with this method. We aimed to compare the temperature measurements by the smartphone application with the axillary digital thermometer.
METHODS: This was a cross-sectional observational study. All patients who were admitted to the pediatric emergency department between 1 July-30 September 2017 with fever were included. Demographic characteristics and the diagnosis of patients were recorded. The temperatures that were measured using smartphone application and axillary digital thermometer were compared.
RESULTS: During the study, 234 measurements were ensured from 53 patients. The mean age of the patients was 58±51 months. Thirty (57%) patients were male and eleven (21%) were infants. The most frequent diagnoses of patients were upper respiratory tract infection (20, 38%) and acute gastroenteritis (12, 23%). There was no significant difference between smartphone application and axillary digital thermometer in detecting fever (p=0.754). The mean measurements of the axillary digital thermometer and SPA were 37.07±0.85 oC and 37.16 ± 0.89 oC respectively. Both measurements were strongly correlated with each other (r=0.874, p<0.001). In the fever period, the mean measurements of the axillary digital thermometer was 38.14±0.56 oC, while smartphone application was 38.24±0.55 oC. These values were also strongly correlated with each other (r=0.745, p<0.001). Similar results were also achieved in infantile period.
DISCUSSION AND CONCLUSION: The Smartphone application seems to be a superior method that can be used reliably, anywhere and in all age groups, however, it needs further studies to introduce this method into routine use.
