Peptic Ulcer Perforation in a Child: Case Report

Since the use of pediatric endoscopy, the diagnosis of pediatric peptic ulcer disease has recently been increasing. Although a perforated peptic ulcer has been reported in approximately 10% of adults with peptic ulcer disease, perforated peptic ulcer in children is rare. Diagnosis can be made by the history, physical examination and abdominal radiography parely, further analysis is required. The 6 year old child who was operated on for a perforated appendicitis had been diagnosed with missed perforated peptic ulcer. Peptic ulcer perforation frequency has gradually increased in the pediatric group. We report this case here because we wish to emphasize that peptic ulcer perforation should be taken into consideration in the differential diagnosis of acute abdominal pain.

Pediatrik endoskobun kullanılmaya başlanması ile birlikte çocuklardaki peptik ülser hastalığının tanısı son günlerde artmaktadır. Erişkinlerdeki dispeptik yakınması olan hastalarının yaklaşık %10 unda peptik ülser perforasyonu görülmesine karşılık, peptik ülser perforasyonu çocuklarda oldukça nadirdir. Peptik ülser perforasyon tanısı hikâye, fizik muayene ve karın grafisi ile konulur, nadiren ileri incelemelere gerek duyulur. Akut appendisit ön tanısı ile opere ettiğimiz 6 yaşındaki çocuk hastada, gözden kaçan peptik ülser perforasyonununu tespit edildi. Çocuklarda peptik ülser perforasyonun sıklığı gittikçe artmaktadır. Biz burada bu vakayı sunduk çünkü peptik ülser perforasyonu akut batın ayrıcı tanısında göz önünde bulundurulması gerektiğini vurgulamak istedik.
