The diagnostic value of somatostatin receptor scintigraphy (SRS) in the detection of tumors has been assessed in a number of studies. We present a 30-year-old female with a history of eight months cough as well as pain in left shoulder. Radiologic evaluation showed a pulmonary mass and some hepatic lesions, with a pathologically confirmed neuroendocrine carcinoma. A 99mTc-octreotide scan demonstrated a somatostatin receptor (SSTR) positive pulmonary lesion but SSTR negative liver metastases. The present case highlights the significance of a differential uptake pattern by somatostatin receptors in somatostatin receptor scintigraphy (SRS) in patients with neuroendocrine tumors.
